Service Options Using Resources in a Community Environment

Service Options Using Resources in a Community Environment (SOURCE) creates a caring network for at-risk individuals who need supportive assistance to remain in their own homes or communities. This alternative to nursing home placement provides comprehensive, preventative medical support with in-home and community services.

What SOURCE Offers

SOURCE offers an array of skilled and supportive services to help individuals remain in the community. A case manager works with the client to develop a care plan from the following services:

  • Daytime care and supervision in an adult day health center
  • Emergency response services
  • Personal support service or extended personal support service (for assistance with bathing, light housekeeping, shopping, laundry and meal preparation, in home respite)
  • Home-delivered meals
  • Skilled nursing
  • Meals, personal care and supervision in an approved personal care home

Qualifications

All SOURCE enrollees must:

  • Have SSI Medicaid or Public Law Medicaid
  • Be 65 or older, with a chronic medical condition OR under 65 and disabled 
  • Meet the medical and functional criteria for placement in a nursing home
