Approved Training Provider Forms

Approved Training Providers for WIA services are required to submit progress reports on WIA customers enrolled in training programs. The forms below should be used to record information and forward to the assigned Career Advisor.
- WIA Customer Activity and Follow-Up Checklist (Word)
This form is used to record customer status updates, completion of training, attendance and/or problems a customer may be having in training. Complete this form as needed and forward to the assigned Career Advisor. Please forward copies of certificates and/or earned credentials and licenses along with this form.
- Employment Checklist (Word)
This form is used to record customer placement and/or end of training services. Complete this form and forward to the assigned Career Advisor. Please be advised that all employment information will be verified before reported on GWS. Credential, certificate and/or license information will not be reported on GWS unless a paper copy has been forwarded to the Career Advisor for maintenance in the customer file.
- Not Employed Checklist (Word)
This form is used to note that you feel that a customer has exhausted all reasonable chances of finding training-related employment. This form may be used to note that the customer is not likely to find employment due to health, medical and/or other reasons.
Approved Training Providers may request to add additional programs to the Eligible Provider List, or to make changes to current approved programs. All Program Change Requests (Word) and Additional Program Requests (Word) must be submitted on the correct form. All program changes are subject to the Quarterly ITA Committee review. Notification of committee recommendations will be provided in writing following the quarterly meeting.
