Support Options

Support Options is a program that offers you an opportunity to direct your own care and choose the services you need to allow you to stay in your home and your community.  Working within an established budget, you will choose the type of services you need and who can provide those services.  You will have access to a fiscal intermediary service that will help you with managing your funds, including billing and disbursement activities as well as employee-related requirements.  You will work with a Support Options Counselor who will help you develop a support plan and assist you in this process.

The following is a list of Support Options service categories:

In-Home Services which may include personal care, homemaker, companion services;
Respite Care which may provide support to the caregiver;
Treatment and Training which may include therapies, family counseling and training, and caregiver training and education;
Environmental Modifications and Provisions which may include home-delivered meals, supplies or equipment, home modifications, and transportation;
Consumer-Direction Support Services which includes fiscal intermediary services.

Who can provide services?

• Family, neighbors, friends, or other community members
• Professional individuals or agencies that you select

Eligibility and Costs

In order to qualify for Support Options you must:
• Be age 60 or older;
• Have impairments that meet the frailty level required for nursing home placement
• Have a total individual income (i.e. Social Security benefits, pension) not to exceed 
  $1,911 per month;
• Have resources that fall within the following range:
 • Single: $12,000 - $39,000
 • Couple: $116.400 - $143,400
• Have needs that can be safely met by Support Options.

Based on your income, you may be expected to pay a portion of the cost for the program.
