Community Care Services Program is a Medicaid-waiver program designed to provide home and community-based services to people who are functionally impaired. The program helps eligible recipients remain in their own homes, the homes or caregivers or in personal care homes as long as possible.

CCSP offers an array of skilled and supportive services to help individuals remain in the community.  A care coordinator works with the client to develop a care plan from the following services:

  • Daytime care and supervision in an adult day health center
  • Emergency response services
  • Personal support service or extended personal support service (for assistance with bathing, light housekeeping, shopping, laundry and meal preparation, in home respite)
  • Home-delivered meals
  • Skilled nursing
  • Meals, personal care and supervision in an approved personal care home
  • Consumer Directed Care

CCSP offers community based services to persons who:

  • Meet the medical and functional criteria for placement in a nursing facility
  • Are eligible or potentially eligible for Medicaid
  • Have health and personal needs that can be met safely in the community and within established cost limits

Individuals may be expected to pay a portion of service cost, based on the applicant's financial situation.
